Certifique-se de que tenha seguidos os procedimentos corretamente, o código funciona, pode conferir no meu fórum de testes:

Tem de editá-lo ao seu modo, alterando o título que irá aparecer na aba do navegador e os respectivos links. Caso você não saiba editar, diga quais são os links que eu monto a página.
